Service Description
1. Sentinel Event Debriefing Objective: Conduct thorough debriefs after any critical incident or sentinel event (e.g., a serious injury, security breach, or near-miss) to analyze what happened, identify contributing factors, and develop strategies to prevent future occurrences. Components: Incident Documentation: Create a structured process for documenting the event, including what occurred, how it was handled, and immediate outcomes. Multidisciplinary Debriefing: Hold a structured, non-blaming debriefing session with involved staff, leadership, and relevant departments (e.g., HR, security, legal) to discuss observations, concerns, and lessons learned. Psychological Support: Provide access to mental health resources, such as counseling or support groups, for employees who may be affected by the event. Root Cause Analysis (RCA): Implement RCA to uncover underlying issues that contributed to the event, including process, communication, or training gaps. Action Plan and Follow-Up: Develop and communicate an action plan to address identified weaknesses, including timelines for implementing changes and follow-up debriefs to assess progress. Outcome: Increased transparency and improved protocols, leading to a safer workplace with a proactive approach to learning from incidents.
